- Embrace digital transformation – expanding online triage, telephony, and self-management tools for convenience and timely access.
- Champion health equity – tackling Core20PLUS5 priorities, PSA screening, and targeted interventions for vulnerable groups.
- Grow and develop our workforce – supporting clinical supervision, training, and student placements to ensure sustainability.
- Promote collaboration – strengthening partnerships with PCN colleagues, FutureMeds, social care, and community organisations.
- Foster a supportive culture – where staff thrive, patients trust us, and the wider community sees us as a hub for health and wellbeing.
