NHS Health Checks
For any patients who are aged between 40-74 years old who have not been previously diagnosed with any of the following conditions:
- Heart disease
- Chronic kidney disease
- Diabetes
- High blood pressure (hypertension)
- Atrial fibrillation
- Transient ischaemic attack (TIA)
- Familial hypercholesterolaemia (inherited high cholesterol)
- Heart failure
- Peripheral arterial disease
- Stroke
- Currently prescribed statins to lower cholesterol
- Previous assessments indicating a 20% or higher risk of developing cardiovascular disease over the next 10 year done every 5 years.
Over 75’s
Patients 75 years and over who have not had a consultation within the last 12 months may request a health review with one of our practice team.
If you are unable to attend the surgery, a home visit could be arranged.
