The provision of private letters is not covered by the NHS, it is private work. Just like private work from a solicitor or any other professional, there is a charge for this work.

- Medicals for Sports/Extreme Sports
- “Fitness to” undertake activities/fly
- Power of Attorney Documents
- Witness to Power of Attorney Application
- Driving Licence application signature
- Passport countersignature
- Camp America Forms
- Confirmation of mental capacity
- Court exemption report – to exempt patient from appearance as a witness (unless requested by the Court)

Access to medical records under UK GDPR

Adoption and fostering

Driving
- DVLA – driving licence questionnaire no examination – £40

Insurance
- Life Insurance report (no examination, paid for by the Insurance Company)- £100

These fees will be reviewed on a yearly basis.
If you require a Non NHS letter or completion of a form from a GP you will
need to complete a letter pack and pay £10 deposit. You can collect a letter pack from the surgery.
